IDDI's Social Services Department provides case management to each resident. Case managers follow the progress of each resident across settings, participate in his/her treatment planning, and facilitate the resolution of problems or issues which may arise. Our goal is to ensure that the needs of each individual are met.
The department serves as the primary liaison between the Children's or Adult Program and families and agencies. Calls from families or other involved or interested parties to address questions or concerns on a wide variety of issues are welcomed. Open communication is always encouraged so that we may provide the best possible services to all of our residents.
We assist with the maintenance of ancillary services and provide documentation required for continued eligibility for both Medicaid coverage and Social Security benefits. We also provide assistance to families or other individuals involved in the guardianship process as our young people approach the age of legal maturity.
